The goal here is to try to record your regular eating habits. I know it’s tempting, but you don’t need to try to be “good” or “healthy” just because I’ll be reviewing it. This is the time for brutal honesty!

Please record beverages, including the approximate amount of ounces consumed. As well, please make a note of bowel movements.

I suggest you print these forms and keep them in your kitchen to fill out by hand over the next three days. Once you are finished, you can scan and email them to me, or you can use the online form below to input what you’ve recorded.
